Our personal assistant program provides our patients with the respect, dignity and compassion they deserve, and is designed to ensure that their independence and quality of life are maintained.
Primary Home Care (PHC)

Personal Care Services (PCS)

Eligibility Criteria
Patient must be confined to his/her place of residence
Ability to understand and ensure that Expectation of medical needs are adequate to be met in his/her residence
Patient must be under the care of a certified physician
Service must be deemed reasonable and necessary according to Medicare, Medicaid and/or insurance
